Your data

At your first appointment I will ask you to complete a consultation form. This will include your name, date of birth, email address, phone number and relevant health history.

I need to keep your personal data so that I can contact you.

It is a legal requirement for me to keep your relevant health information to ensure that I can provide safe and professional treatments for you. It is your responsibility to keep me updated of any relevant medical or health information. In some cases it may be necessary for me to obtain medical information from your G.P. or other health care professional; this will only be done with your written consent.

I will keep written notes of all treatments I provide for you with your consultation form. The information will only be viewed and used by me.

Data Retention

All of your data will be stored securely inside my treatment room.

If you choose to stop having treatments with me. You may ask me to remove some of your data.

In order to meet insurance and financial obligations, I am required to keep certain details on record for a fixed period of time from the date of your last treatment in order to meet the requirements of relevant policies and legislation. At present this is five years for insurance and six years for tax/financial purposes.

In some cases, it may be possible to anonymise some of the data. If this is not possible, I will not delete your details until after that period has passed.
